Since we are on to liver damage, does APAP damage the liver in normal doses ? Because in normal doses the main metabolism pathways produce harmless metabolites and very little NAPQI is made, which gets cleaned out by gluthathione anyway.

That depends on the person's natural level of glutathione. Liver damage from APAP occurs when your glutathione gets overwhelmed, the acetaminophen binds to protein, and the resulting ROS signals a downstream response. Then your body alters its calcium homeostasis, which alters the mitochondrial membrane potential, and prevents the cells from synthesizing enough ATP. The metabolite does not bind to protein until almost all of your glutathione is depleted, though. So it depends on your natural levels as to what dosage will cause damage.
