Until this topic came up, I was not 100% clear on the mechanism either. I had assumed that it's actions were similar to amphetamine and the DAT, due their similar structure. However, it seems their mechanisms are actually very different. Where amphetamine binds to the DAT, enters the neuron, and inhibits VMAT-2 to instigate a release of DA; MDMA is an extracellular presynaptic 5-HT2B agonist, which then instigates a release of 5-HT without actually entering the neuron. It's very interesting!
Since peripheral 5-HT2B receptors control smooth muscle movement, MDMA binding to them in the GI tract could cause bowel movements in a similar fashion to caffeine. I believe that is the mechanism involved in the "MDMA poops."
