So what exactly is Phenylpiracetam supposed to do?

Just took 100mg dosage of it as I just got it on my doorstep. This was about 10 minutes ago. 5 hours ago I took 200mg of caffeine and 400mg of l-theanine. The only thing I feel is that classic placebo effect where things "look sharper and sounds are clearer" and stuff like that. Is that just my head wanting to believe something is happening or is this actually what it's supposed to do? The sources I used to see what it was told me it would "improve cognition" but that's extremely vague.

Is this only true if you take it with choline?

No. You only need to supplement with choline if you are deficient. This usually manifests itself as a headache. The stimulant properties of phenylpiracetam are not from the pyrrolidine structure, but from the phenyl group and its affinities for norepiniephrine and dopamine.
