I don't think the quality is a factor. I think a lot of people have prejudice against any drug that didn't come from a pharma company... But if you read the abstract, it just sounds like these people took overly high doses.
I can imagine it, some dude has never used phenibut, takes one or two grams. Two hours later he doesn't feel it much, so he takes 3 more grams. Then it starts to kick in, and soon enough he's nauseated, confused, has difficulty walking, he passes out. Wakes up the next day, still feels super confused, panics because he thinks he might have caused some permanent brain damage, goes to the ER.

THC mixed with alcohol and phenibut, according to the article. And the only symptoms were:
The main finding was a decreased level of consciousness, however when roused she became delirious. Supportive care only was required with no specific intervention.
So they mixed three psychoactive substances, two of which are GABAergic, and were sedated and psychologically affected. No other complications were mentioned, they did not do anything to treat, and full recovery happened in 24 hours. It was a stupid decision for the person to mix those things, but it just sounds like they were messed up on sedating substances, and someone took them to the hospital freaking out.
Altered mental status was the predominant manifestation of phenibut toxicity in these cases.
Why are they saying toxicity? If altered mental states is the bar set for toxicity, I don't have long to live...
