Why does Grapefruit Juice potentiate so many different drugs?

From morphine and ketamine all the way to caffeine; how come this is the case?

I've heard that white grapefruit juice is best and that juice from concentrate is even better.

Is this something which has been isolated and made available as a supplement / whatever it would be?

Say I took 150mg of pure MDMA with and without grapefruit juice. Would the amount of neurotoxicity be the same with both? Or would the one with the grapefruit be more neurotoxic because more MDMA is being absorbed?

Less neurotoxic. MDMA's 5-HT toxicity arises from it's metabolites. Most of the damaging ones actually come from MDA, which is created when MDMA is metabolized by you CYP3A4 enzyme. Grapefruit juice inhibits this enzyme, so it lowers toxicity and increases effects. Someone already linked my post on the matter. Take a look through it.

Now taking grapefruit juice with MDMA will lower 5-HT system damage, but can raise the risk of other damage like serotonin syndrome or excitotoxicity. This is because it will cause your plasma concentrations of MDMA to be higher. This is only an issue if you are taking large amounts. If you are being reasonable, grapefruit juice lowers toxicity while increasing effects. I don't roll without it.
