What exactly are we to beware with grapefruit juice? Are you saying the CYP3A4 inhibition is bad? I just took piperine with my modafinil, and that is a much stronger CYP3A4 inhibitor. It definitely potentiates the modafinil very nicely, and eliminates some of the adverse side effects.
Another drug that benefits from CYP3A4 inhibition is MDMA. Preventing the N-demethylation to MDA significantly mitigates the 5-HT system toxicity, as MDA's metabolites are much more damaging than MDMA's. Then you also have other drugs like opiates that are potentiated by CYP3A4 inhibition.
I would say be aware, rather than beware. I know it's semantics, but CYP3A4 inhibition can be useful in many cases. Also, grapefruit juice does not inhibit liver CYP3A4 levels very much. It's mainly limited to the GI tract. You will need something like piperine to really inhibit liver enzyme levels.
