Issue with out of stock Notifier form

Dear Nootropics Depot,
I am having an issue with the out of stock notifier form on your website. I am trying to be notified when more ND 24% Bacopa Extract Tablets are in stock, but every time I fill out the form with my email, it just pops up a dialog box that says "Please select all options". I can't figure out which options I am not selecting.
Also, I was wondering if you knew when more of your in-house 24% Bacopa Extract Tablets would be in stock?

Do you have any timeline for when your in-house Bacopa Extract will be back in stock? Specifically the tablets? I understand if you aren't able to answer this. Thanks .

No firm ETA. Supply chain out of India has been a mess this year. Every recent batch we have gotten in has failed our testing. The most recent one only failed by like 1%, but I don't want to just modify our spec out of convenience. We are working on a resupply, but I can't give any estimates yet.
