Issue with out of stock Notifier form

Dear Nootropics Depot,
I am having an issue with the out of stock notifier form on your website. I am trying to be notified when more ND 24% Bacopa Extract Tablets are in stock, but every time I fill out the form with my email, it just pops up a dialog box that says "Please select all options". I can't figure out which options I am not selecting.
Also, I was wondering if you knew when more of your in-house 24% Bacopa Extract Tablets would be in stock?

That's just asking you to select the size you want to be notified for. It's a bit weird because the options are greyed out. However, if you select one of the sizes, then you can click the button and have it notify you when it gets back in stock. I will check with my team to see if there is a way it can automatically select the small size if none is selected, though. I admit that it is a bit confusing.
