How are nootropics made?

Can someone explain to me how nootropics are made? Drugs like piracetam, modafinil, etc... Do chemists just take mole ratios and combine them? I know that that isn't how they do it but could someone clarify this subject for me?

To be serious, what you are asking is complex. Nootropics are not just one thing, and are made very differently depending on the specific compounds.
