What nootropics should not be mixed?

I'm trying to be cautious, so I was hoping to find out if there's anything I shouldn't be mixing together.

Do not take memantine and D-cycloserine. I can honestly say that was the worst brain fog of my life. I could barely walk, and could not hold onto thoughts for more than a few seconds. Lowering the activity at the glycine binding site, combined with the NMDA antagonism of the memantine, proved to be too much inhibition.
