More like careful combining MAO-Is and serotonergic drug. Methylene blue could never have serious adverse effects when taken in nootropic doses, as it does not cause MAO inhibition until dosed orders of magnitude higher.

Cases were classified as severe if reintubation with mechanical ventilation was required for more than 24 hours, or if patient recovery took longer than 96 hours. There was no correlation between the dose of methylene blue and the severity of encephalopathy. One death was reported, at the time presumed by the authors to be a result of the patient's comorbidities.
The doses were from 1.75mg/kg and 7.5mg/kg. The bolded text would allude to the fact that the dosage was not a factor. I'll have to do some more reading.
