I contacted the Amazon store yesterday about a certificate of analysis and it looks like s/he recently uploaded it on the product page. Based on the watermark the methylene blue looks to be from Macsen Laboratories, which advertises itself as a GMP, GLP and ISO certified lab and the world's largest manufacturer of pharmaceutical grade methylene blue.
Maybe our best bet is ordering methylene blue directly from Macsen Laboratories? Let me know if you guys have any ideas.

Ahh India. Now that begs the question: is this anhydrous or trihydrate? There is no CAS on the COA. It says methylene blue, rather than Methylthioninium Chloride. So that probably means it is anhydrous. None of the studies I can find denote the type used.
