TMAO is created by gut bacteria metabolizing things like carnitine and choline in the large intestine. The issue with taking normal L-carnitine is that only 14-18% gets absorbed through passive transport, with the rest being broken down by the gut microbiome. That's where the TMAO can be created. ALCAR is able to absorb through active transport, though. This should result in less risk of your microbiome metabolizing it into TMAO. Also, taking berberine and vitamin D can help lower TMAO, so you can take those alongside ALCAR.
