I know that long-term use of prescription mTOR inhibitors can cause muscle wasting. That would be counterproductive for underweight people, for sure.

Sometimes less is more. Start at 250mg. Also, some people have noted that Cistanche makes them hungry. I don't personally notice it, but multiple people have relayed that to me so far.
